Fire Prevention Form
The City of Gilroy uses several forms in its Fire Prevention Program.

Construction Permit Application Form
The Fire Prevention Program uses the same application form as the Building Department for any construction or installation activity that requires a permit. This form is to be submitted with plans and specifications for the installation to the BLES permit counter at City Hall.

Application Form for Fire Permit for Occupancies
For Occupancies requiring a permit for a use, such as welding or cutting, there are 2 forms used, Business Activity Notification Form (PDF) and the Owner / Operator Form (PDF). These forms are also used for the Hazardous Materials and Industrial Waste Programs. These forms are completed and submitted with any floor plans or other required information.

Fire Safety Inspection Form
This basic fire inspection form is used to document fire safety during routine inspections.
